Living with a chronic condition can affect your energy, confidence, and quality of life. At True North Wellness, our Accredited Exercise Physiologists provide safe, personalised exercise programs designed to help you manage symptoms, improve function, and feel more in control of your health.

We work alongside your GP and healthcare team to support long-term health — not quick fixes.

How Exercise Physiology Helps Manage Chronic Disease

Exercise physiology is a clinical allied health service focused on using exercise as medicine.

Accredited Exercise Physiologists (AEPs) are university-qualified professionals who specialise in working with people who have chronic, complex, or long-term health conditions.

We help you:

  • Improve strength, mobility, and cardiovascular health
  • Reduce pain, fatigue, and breathlessness
  • Improve blood glucose control and metabolic health
  • Build confidence to move safely
  • Maintain independence and quality of life

Every program is individualised, evidence-based, and adjusted to your condition, symptoms, and goals.

  • Goal setting that reflects what matters to you
  • A personalised exercise program tailored to your condition
  • Education on pacing, symptom management, and safe progression

Sessions can be delivered one-on-one or in small supportive groups, depending on your needs and preferences.

Accessing Exercise Physiology Through Medicare

You may be eligible for Medicare rebates if you have a chronic condition.

If your GP has prepared a Chronic Condition Management Plan, you may be able to access rebates for up to five allied health sessions per calendar year, including exercise physiology.

How it works:

  1. Visit your GP and discuss a Chronic Condition Management Plan
  2. Ask for Exercise Physiology to be included
  3. Bring your referral to True North Wellness

We can also see you without a referral, and private health rebates may apply depending on your cover.
