    Long Term, Sustainable Change

People who have the greatest success in making sustainable changes usually have weekly or fortnightly appointments for 1-2 months and then have either monthly, or 3-monthly reviews after that, dependant on their unique goals and needs. We have some introductory and review packs that may allow you up to 15% savings on your nutrition support. Sessions will look different dependant on who you are and what you want to achieve. If you’re not sure which pack to choose, we recommend you start with an Intro Pack, which will set you up for the first 1-2 months..

    Introductory Dietitian Packs

    Initial Appointment

In the initial appointment we will assess your diet and any medical or lifestyle factors relevant to your situation. This appointment usually goes for 50-60 minutes and involves a lot of conversation (you will do most of the talking). We will support you to develop personalised goals, and if relevant give you some basic introductory education relevant to your goals.

    Second Appointment - Building Strategies

In the second appointment, we will look at developing personalised strategies to support you to achieve your goals. This can include providing education or resources and developing a personalised plan for what you will do to achieve your goals over the coming 1-2 weeks.

    Third Appointment - Making Changes

Some people make changes very fast and will have already implemented some of their plan, others will find that the original plan didn’t work and needs adjustment. This appointment focuses on reviewing the plan and adjusting it as required. We will provide additional education and resources as needed to support you at this appointment.

    Fourth Appointment - Reviewing Progress

By the fourth appointment, most people have had a go at making some changes. This appointment gives us a chance to look back at where you started and review your progress to see what is and isn’t working. This can include taking measurements, or checking back in on the initial discussions from the initial appointment.

    LIFESTYLE CHANGER PROGRAM

    CHRONIC DISEASE REFERRAL

individuals with chronic conditions (present for 6+ months) can access a Chronic Disease Management (CDM) Plan through Medicare, which allows them to receive services from allied health professionals, including Accredited Practising Dietitians (APDs). Chronic conditions may include (but are not limited to) diabetes, cardiovascular disease, chronic kidney disease, arthritis, asthma, and obesity.

Under the CDM Plan, patients are eligible for up to 5 Medicare-subsidized services per calendar year.  The number of sessions with each allied health provider depends on the patient's needs and the GP’s recommendations (e.g., you might use all 5 sessions with a dietitian or split them between different allied health). Medicare rebates cover a portion of the cost of each session. Ask your GP whether this may be applicable for you.

    EATING DISORDER REFERRAL

Individuals with eating disorders may be eligible for support under Medicare through the Eating Disorder Treatment Plan (EDTP). This plan provides access to a range of treatment services, including dietetic consultations, psychological services, and medical management, designed to help people with conditions like anorexia nervosa, bulimia nervosa, binge-eating disorder, or other specified feeding and eating disorders.  It can provides for up to 20 dietetic sessions (with an Accredited Practising Dietitian) per calendar year under Medicare.

A GP, psychiatrist, or pediatrician can assess and determine if an EDTP is appropriate, so ask your doctor whether this may be applicable for you.

Carbs are not bad for you.

Despite current conversations around diet, carbohydrates (AKA carbs) are a very important part of healthy, balanced diets. Including the right types of carbohydrate foods in your diet is the key to maintaining energy levels throughout the day, which can improve your focus and overall health and wellbeing.
