Who is this for?

Our cardiac rehab and heart health services are designed for people who have:

  • Experienced a heart attack, heart failure or undergone heart surgery
  • Diagnosed cardiovascular conditions (e.g. hypertension, arrhythmia, angina, cardiomyopathy)
  • Undergone angioplasty or stent procedures
  • High blood pressure, high cholesterol or family history of heart disease

What to expect

We work alongside your cardiologist or GP to deliver evidence-based, structured support.

Your cardiac rehab journey may include:

  • Initial assessment: Review of medical history, cardiac risk factors, and current fitness

  • Tailored exercise prescription: Safe, progressive movement plan adapted to your health status and goals

  • Education and support: Learn how exercise improves heart function, helps control blood pressure, reduces cholesterol and supports long-term heart health

  • Monitoring and guidance: Supervised sessions (in-person or remote) with heart rate and symptom monitoring, where appropriate

  • Lifestyle advice: Support around physical activity, stress management, and sustainable health behaviours

Already Completed Hospital-Based Cardiac Rehab?

You may be ready for Phase 3 cardiac rehab: community or home-based programs that bridge the gap between clinical recovery and long-term health. We’ll help you maintain momentum and make movement a regular, safe part of your life.

Benefits of working with an AEP for cardiac rehab

  • Improved cardiovascular fitness and endurance
  • Reduced risk of future heart events
  • Better control of blood pressure, cholesterol and weight
  • Enhanced energy, mood, and confidence
  • Safe return to daily activities and exercise
